Functional Medicine for Chronic Fatigue
Fatigue is one of the most common reasons people visit their doctor, and one of the most frequently dismissed. Labs come back normal. Thyroid looks fine. Blood count is in range. The advice is to sleep more, stress less, and exercise.
For many people, the problem isn’t that nothing is wrong. It’s that the right questions haven’t been asked yet.
Why Am I Always Tired? The Limits of Standard Testing
Standard blood panels are designed to catch established disease, not the early imbalances that drain energy before a diagnosis is warranted. A TSH in the normal range doesn’t tell you how well the thyroid is actually converting hormones. A fasting glucose within range doesn’t reveal whether insulin resistance is already affecting how your cells use fuel. A CBC with no anemia doesn’t rule out the kind of nutrient insufficiencies that quietly erode energy over months or years.
This is the gap functional medicine works in. Not the absence of disease, but the presence of dysfunction that hasn’t crossed a diagnostic threshold yet.
What Are the Root Causes of Fatigue?
Persistent tiredness that doesn’t resolve with rest, that varies across the day without obvious reason, or that comes with brain fog, disrupted sleep, or low mood is the body signaling that something in its underlying systems is off. Finding what that is requires a different kind of investigation.
Fatigue rarely has a single cause. More often, it reflects a combination of factors that reinforce each other. A functional medicine doctor for chronic fatigue approaches the problem by examining several interconnected systems rather than looking for one diagnosis to explain everything.
How Functional Medicine Investigates Fatigue
Thyroid dysfunction. The standard thyroid test, TSH alone, can miss significant problems. A full thyroid panel including free T3, free T4, reverse T3, and thyroid antibodies gives a more complete picture of how the thyroid is producing, converting, and using hormones. Subclinical hypothyroidism and Hashimoto’s thyroiditis are among the most commonly missed contributors to fatigue in patients told their thyroid is “normal.”
Adrenal and cortisol patterns. The body’s stress response system, centered on cortisol, follows a daily rhythm. When that rhythm is disrupted by chronic stress, poor sleep, or prolonged illness, the result is what many clinicians call HPA axis dysregulation: cortisol patterns that don’t match what the body needs. Morning exhaustion despite a full night’s sleep, an afternoon energy crash, and feeling wired at night are classic signs. A four-point salivary cortisol test maps this pattern in a way a single morning cortisol blood draw cannot.
What Does Chronic Fatigue Treatment Look Like?
Nutrient deficiencies. Iron deficiency can cause significant fatigue at levels well below those that produce clinical anemia. The same is true of vitamin B12, vitamin D, magnesium, and ferritin. Standard panels often don’t include all of these. And even when they do, the “normal” reference ranges are based on population averages, not on what’s optimal for any one person. A vitamin B12 result of 210 pg/mL is technically within range, but many clinicians consider anything below 400 functionally insufficient. Someone at 210 may be experiencing real fatigue and brain fog while being told their levels are fine.
